You may see an unfamiliar test result entry within your NHS/Patient Access App called John Hopkins adjusted clinical groups system score.  This is to help our team identify and prioritise more vulnerable patients and is nothing to be concerned about.

If you have an NHS Log in, you can use that (a single login that securely gives you access to Health websites and apps with just one username and password).

If you don't have an account you will need to Create one first

This is on the right hand side of the screen if using a PC or below if using a ipad/phone

 

 

If you already have an account sign in.

 

REMEMBER, whichever way you choose to access Patchs, you will need to access that way for all future Patchs requests.

 

If you choose the NHS Login you can then access Patchs via your NHS App, by clicking on Advice, then click on Ask your GP for medical advice.

Then click on Continue to Patchs.

 

Make sure you answer all questions and get the message at the end that says 

"THANK YOU WE HAVE RECEIVED YOUR REQUEST"

 

The request will NOT be submitted unless you receive this message.
